肾病综合征的肾血流与病理类型的关系

摘　　要：目的　探讨彩色多普勒超声对肾病综合征 (NS)的肾血流动力学改变特征及病理类型间的关系。方法　将临床诊断、肾穿病理证实的 6 9例系膜增生性肾炎的病人 ,根据病理分轻、中、重三组 ,并与正常健康者 4 0例为对照组比较 ,观察肾段动脉、叶间动脉的收缩期最大流速 (Vs)、舒张期末速度 (Vd)、平均速度 (Vm)、搏动指数(PI) ,阻力指数 (RI)参数。结果　 NS病人的肾段动脉、叶间动脉的 Vs、 PI、 RI出现显著性差异 (P<0 .0 1、 P<0 .0 5 ) ,但在病理各分型中血流差异不明显。结论　彩超对 NS病人的肾血流改变特点和估测病情变化。Objective To study changing characters of renal henodymanics of nephrotic syndrome(NS),and the relation between these characters and pathological types by color Dopper Ultrasonography(CDUS).Methods Having been proved by clinical and renal biopsy pathology,69 cases with membranoproliferative nephritis were divided into three groups(slight degree,moderate degree and severe degree)according to pathology.To measure below parameters of renal artery and interlobar renal arteries:Peak velocity in systole(Vs),end diastolic velocity(Vd),averge velocity(Vm),pulse index(PI) and resistance index(RI).Reusults The results showed that obvious difference was found in Vs,PI,RI of renal artery and interlobar renal arteries of NS(P<0 05 or P<0 01),but no obvious difference was showed among pathological types.Conclusions CDUS provide a non injured way in measuring changing characters of renal blood flow and assessing disease changes and prognosi for NS.
