内镜下氩离子凝固术治疗消化道息肉的临床价值

摘　　要：目的　探讨内镜下氩离子凝固术 (氩气刀 )治疗消化道息肉的价值。方法　采用德国ERBE公司生产的APC3 0 0EA型内镜专用氩气刀对经病理学证实的消化道息肉进行氩离子凝固术。结果　 74例患者共 116枚息肉 ,其中 10 6枚为扁平、广基息肉 ,经内镜氩气刀治疗 ,另 10枚有蒂息肉在内镜圈套治疗时采用内镜下氩气刀配合治疗 ,全部治愈。每枚息肉根据其部位、大小决定氩离子凝固次数 ,一般经 2～ 4次凝固息肉皆变平 ,每次内镜最多可处理 10枚息肉 ,息肉超过10枚者宜分次进行治疗。结论　内镜下氩气刀对消化道广基、扁平息肉或息肉样隆起可作为首选的治疗方法 ,其疗效佳 ,安全性好 ,副作用少 。Objective To evaluate the values of the endoscopic argon plasma coagulation (APC) for the treatment of gastrointestinal(GI) polyps. Methods The GI polyps confirmed pathologically were coagulated by argon plasma generated by ERBE APC 300 EA made in German ERBE Company. Results A total of 116 polyps, including 106 flat and thin sessile polyps, 10 pedunculated polyps, in 74 cases were all cured by APC. Time of coagulation for every polyp was determined by its size and location. Polyps became flat and thin after 2-4 times of coagulation. As many as 10 polyps could be coagulated at one time, but for patients with more than 10 polyps, several times of coagulation were needed. Conclusion Treatment of GI polyps by APC is of safety, satisfactory therapeutic efficacy and low side effects. It can be the first choice of methods for the treatment of sessile gastrointestinal polyps, flat and thin polyps or polypoid projection.
