摘 要:目的 :探讨应用彩色多普勒超声监测甲状腺动脉栓塞术前、术后 ,甲状腺的声像学特征及血流变化规律的意义。方法 :10例难治性甲亢患者行甲状腺动脉栓塞术 ,分别于术前、术后 1个月、术后 3个月对患者进行彩色多普勒超声监测 ,观察甲状腺的大小、实质回声、血供及各血流参数的变化情况。结果 :术前甲状腺腺体弥漫性肿大 ,血供丰富 ,呈“火海征” ,各血流参数明显升高 ;术后甲状腺腺体缩小 ,血供明显减少 ,各血流参数明显降低。术后 1个月与术前比较 ,各项监测内容均存在显著性差异 (P <0 0 5~ 0 0 0 1) ,术后 3个月与术后 1个月比较 ,各项监测内容继续有所变化 ,但差异不显著 (P >0 0 5 )。结论 :彩色多普勒超声对甲亢甲状腺动脉栓塞术后的疗效观察 。Objective:To investigate the acoustical characteristics and thyroid blood flow changes before and after the thyroid arterial embolization treatment for refractory hyperthyroidism by color Doppler ultrasound.Methods:Ten Patients with refractory hyperthyroidism treated with thyroid arterial embolization were analyzed.Color Doppler ultrasound was applied to evaluate the thyroid size,echoes,vascularities and the changes of blood flow parameters before operation,1 month and 3 months after operation.Results:Befroe operation,the thyroid glands were diffusely enlarged and hypervascular(fire sea sign)and all blood flow parameters were obviously increased.A few days after arterial embolization,the size and vascularity of the thyroid and the blood flow parameters were reduced markedly.All the parameters had significant differences between the preoperation and 1 month post operation( P <0 05~0 001).There was no significant difference between 1 and 3 months after operation( P >0 05)though the parameters of both were changed.Conclusions:Color Doppler ultrasound plays an important role in clinical follow up and for assessing the outcome of the arterial embolization treatment for refractory hyperthyroidism.
