新药静注型卷曲霉素治疗复治菌阳肺结核远期效果观察

摘　　要：目的 :了解新药静脉滴注型卷曲霉素对复治菌阳肺结核病人的治疗效果。方法 :选复治菌阳肺结核病例 ,按 2 :1比例随机分入试验组和对照组 ,采用含卷曲霉素、异烟肼、利福平、吡嗪酰胺及乙胺丁醇等药物方案 (2CHRZE/6H3 R3 E3 )分别进行治疗。结果 :静滴型卷曲霉素试验组强化期末与满疗程结束时其痰结核菌阴转率分别为 70 .5 %、83 .3 3 % ,卷曲霉素对照组强化期末与满疗程结束时痰菌阴转率为 69 6%、80 82 % ;四年间试验组与对照组复发率分别为 4.8%、5 .5 % ,静滴型卷曲霉素组复发率低于对照组 ;不良反应发生比例分别为 2 1.8%和 47.1%。结论 :静滴型卷曲霉素治疗复治菌阳肺结核临床应用效果肯定 ,复发率低 ,避免了注射部位的硬结、疼痛 ,减少了患者痛苦 。Objective:To evaluate the longtime therapeutic effect for tuberculosis with positive sputum cured by intravenous Capreomycin Sulfate. Methods: The tuberculosis with positive sputum were randomly divided into two groups test group (intravenous) and control group (intramuscular). These were given the chemotherapy (2CHRZE/6H 3R 3E 3) containing capreomycin, isoniazid, rifampin, pyrazinamide and ethambutol. Results: The negative conversion rate of sputum in the end of the finish of therapy course were 83.33% and 80.82% for two groups respectively. The tuberculosis reinfection rate of the case cured during the four years were 4.8% and 5.5% for two groups respectively. The incidence of adverse drug reaction were 21.8% and 47.1% respectively. There was significant difference in two groups ( P <0.01). Conclusion: The therapy efficacy of test group was higher than control group, but the tuberculosis retreated rate was lower. The capreomycin sulfate used by intravenous is better than intramuscular for the tuberculosis retreated patients with positive sputum.
