摘　　要：目的 :探讨儿童肘外翻畸形的发生机制、预防和治疗方法。方法 :8例儿童肘外翻畸形 ,1例非手术治疗 ,2例尺神经炎者行神经松解前移术 ,5例创伤性关节炎者行肱骨髁上截骨术。结果 :所有手术病例均进行随访 1.5～ 5年 ,平均随访 3 .4年 ,2例神经松解前移术后症状消失 ,5例肱骨髁上截骨术后获满意外观及功能。结论 :儿童肘外翻畸形有先天性及后天性因素 ,先天性畸形多与性染色体异常有关 ,而后天性畸形大多为肘部损伤后未能及时治疗或治疗不当所致。肘部损伤早期准确复位、确切固定是预防肘外翻畸形的重要措施。严重肘外翻畸形可行肱骨髁上截骨术 ,合并尺神经炎应早期行尺神经松解前移术 ;出于外观考虑 。Objective:To study the pathological mechanism of cubitus valgus deformity and to probe its prevention and treatment.Methods:8 cases of cubitus valgus deformity were operated.Neurolysis and anterior transposition of ulnar nerve were performed in 2 cases with ulnar neuritis;Supracondylar osteotomy of the humerus were done in 5 cases with arthritis of the elbow.Results:All operated patients were followed-up for 1.5-5 years (mean 3.4 years).2 cases with neurolysis and anterior transposition of ulnar nerve and their sympotoms disappeared,5 cases with supracondylar osteotomy of humerus had satisfactory function and good appearance of the elbow.Conclusion:Cubitus valgus deformity in children include congenital and acquired factors.Most congenital deformities are correlated with sex chromosome abnormalities.The delayed and incorrect treatments after the elbow injury are the main causes for acquired deformities.The elbow trauma should be early and accurately reduced and firmly immobilized in order to prevent cubitus valgus deformity.Supracondylar osteotomy is perfomed in the serious cubitus valgus deformity;and neurolysis with anterior transpoition should be done early in deformity with ulnar neuritis;Apperance of the elbow considered,osteotomy′s indications should be correspondingly expanded.
