哈氏棒和Z-plate钢板在胸腰段椎体结核中的应用比较

摘　　要：目的 :探讨脊柱结核外科治疗中应用内固定的治疗效果。方法 :总结 1999年 3月～ 2 0 0 2年 2月采用病灶清除、椎体前路植骨 ,同时 Harrington棒或 Z-plate钢板固定治疗胸腰段椎体结核各 4例。结果 :经过平均 18.4个月随访 ,患者腰背疼痛好转 ,后凸畸形改善 ,平均矫正 7度 ,术后 10个月骨融合率为 62 .5 %。结论 :植骨同时内固定在重建脊柱稳定 ,加速骨融合中有积极意义。Harrington棒矫正后凸畸形方便而可靠 ,但在撑开椎体间隙安放植骨块 ,确保其安全、牢固方面不易操作。 Z-plate钢板因前路暴露充分 ,病灶清除彻底 ,植骨安放准确 ,再加螺钉固定可产生机械的即刻稳定作用。Objective:To explore the effect of using internal fixation in surgical treatment of spondylocace.Methods:Summexd up 4 cases of spondylocace patients who treated by focal cleaning,anterior aproach implantation of vertebra mean while using Harrington Rod and steel Z-plate fixation to treat thoracolumparal spondylocace.Results:All patients got pain released,rachiokyphosis improve,mean corrective frequency is 7°,after 18.4months fellow-up and the fusion frequency of post operation is 62.5%.Conclusion:OS implantation and inner fixation are significantly benefit to stablity of rebuilding spine,speeding fusion frequency.Harrington Rod is convenient and reliable to correct rachiokyphosis but is difficult tl operate.And steel Z-plate is more complete of focal cleaning,precise to implant the bone and stable to fix the spine.
