摘  要:目的:Nucleus CI24M型人工耳蜗系统的神经反应遥测(neural response telemetry,NRT)测试系统的缺陷之一是由于放大器易于饱和而影响波形的有效采集。本研究的目的是通过采集不同延迟(delay)的反应波形找到最佳的延迟时间,以便有效地避免放大器饱和及N1峰的丢失。方法:24例患者参加了本次试验。测试使用NRT2.04版本软件。波形采用削减算法(subtraction method)提取,分别测试24例患者在不同延迟条件下的反应波形并进行分析。结果:锯齿形(jagged)波形大多出现在延迟为35-40μs之间。延迟在40-120μs之间时有超过半数的患者可记录到N1峰,而在50-90μs之间时则有80%以上的患者可记录到。当延迟超过300μs时,Ⅲ型波形的出现率随延迟时间的增加而增加。结论:延迟时间的选择会显著地影响NRT反应波形中N1峰的记录。使用较短的延迟时间进行采样会引起放大器饱和。延迟时间过长会造成N1峰的丢失。记录N1峰的最佳延迟时间为50-90μs。Objective: To investigate the affects of the changing of delay time on the neural response telemetry (NRT)test for cochlear implant patients. One of the limitations of NRT system of the Nucleus C124M device is saturation of amplifier which affects the recording of NRT responses. The purpose of this study is to find optimizing delay to avoid amplifier saturation and Nl peak missing by recording NRT responses in different delays time. Methods: Twenty-four patients were treated by surgery of Nucleus CI24M cochlear implant from August 2000 to August 2001. There were males and females,ranging in age from 2 to 21 years old. The NRT 2. 04 software was used in this test and the subtraction method was used to record NRT responses of 24 patients. Results:The 'jagged' shape of NRT response wave was obtained in the delays time from 35 to 40μs. The Nl peak was recorded about half patients with delays from 40 to 120μs. When delays time was above 300μs,more patients could be recorded wave of type Ⅲ ,and appear rate of Ⅲ wave was increased with delay time increase. Conclusion:Result showed that the delay time was great influence on recording Nl peaks of NRT responses. When short delays time is using, the amplifier often appears saturation. The Nl peak will miss with longer delays. The optimizing delays to record Nl peak are from 50μs to 90μs. [Chinese Arch Otolaryngol head Neck Surg,2003; 10(1):3-5
