摘　　要：目的:随访分析第一跖骨远端Chevron截骨联合远端侧方软组组松解治疗中重度踇外翻畸形的近期临床疗效。方法:随访2010~2014年间通过第一跖骨远端Chevron截骨联合远端侧方软组组松解术治疗的中重度踇外翻20例20足,术前术后摄负重位X片,测量手术前后踇外翻角(HVA)、第一、二跖骨间夹角(IMA),使用AOFAS评分评价临床效果。结果:患者术前HVA角34.3°±7.6°,术后HVA角11.3°±5.6°;术前IMA角16.2°±2.7°,术后IMA角7.6°±2.0°,术前AOFAS评分为55.7±13.6,术后为91.8±8.0,皆具有统计学差异(P<0.01),所有病例无跖骨头坏死、无骨折不愈。结论:第一跖骨远端Chevron截骨联合远端侧方软组组松解术治疗中重度踇外翻效果良好。Objective: To evaluate the clinical outcome of distal Chevron osteotomy with distal lateral soft tissue release for moderate to severe hallux valgus. Method: 20 patients of 20 feet with moderate to severe hallux valgus were operated with distal Chevron osteotomy with distal lateral soft tissue followed up between 2010 and 2014.The hallux valgus angle and first- second intermetatarsal angle were measured on the X- ray film. The clinical outcome was evaluated with AOFAS system. Results: At follow- up,the pre- operative hallux valgus angle was34. 3° ± 7. 6°and the post- operative hallux valgus angle was 11. 3° ± 5. 6°. The pre- operative first- second intermetatarsal angle was 16. 2° ± 2. 7°and the post- operative first- second intermetatarsal angle was 7. 6° ± 2. 0°.The pre- operative AOFAS score was 55. 7 ± 13. 6,and the post- operative AOFAS score was 91. 8 ± 8. 0; and( P < 0. 01) for all. All the results showed a significant difference. Conclusion: The study suggests that the short- term clinical outcome of distal Chevron osteotomy with distal lateral soft tissue release for moderate to severe hallux valgus is satisfactory.
