中老年胆系疾患手术290例临床分析

摘　　要：目的　探讨中老年胆系疾患的特点及胆系手术风险与预后。方法　对我院 1995年 5月至2 0 0 2年 5月收治的 2 90例中老年胆系疾患手术病例进行回顾分析。结果　此组病例主要是胆系结石伴炎症 ,择期手术 2 81例 (96 9% ) ,急诊手术 9例 (3 1% ) ;有合并症 180例 (6 2 1% ) ,其中合并心血管病和异常14 6例 (5 0 3% ) ,糖尿病 10 1例 (34 8% ) ,肺部异常 38例 (13 5 % ) ;发生手术并发症 4 2例 (14 5 % ) ;治愈 2 82例 (97 2 % )。结论　中老年胆系疾患者合并症多 ,构成手术风险大 ;重视围手术期处理 。Objective To ivnestigate the relationship between the senility and the incidence of the disease of the bile duct system,and the risks and prognosis of operation of senile bile duct system.Methods A retrospective analysis was made in 290 middle and senile patients with bile duct disease admitled to our hospital from May 1995 to May 2002.Results In the group of the patients,most of them were lithiasis companied by inflammatory in the bile duct system.281 patients underwent selective operation at 96.9%,9 patients had emergency operation.180 patients suffered from other diseases in which cardiovascular disease were 146 patients,diabetes mellitus were 101 patients,the pulmonary disease were 38 patients.The incidence of operational complication was 14.5 percent(42/290),the curative rate was 97.2 percent(282/290).Conclusion The senile pathologic and physiological characteristic lead to high rate of the bile duct disease with high operation risks.Considering the treatment of perioperative period,selecting the proper surgical opportunity and the surgical methods are the keys of improving the surgical effect of bile duct system disease and the prognosis in middle and aged patients.
