超声引导下自动活检装置在胸部疾病诊断中的应用

摘　　要：目的 :探讨超声引导下经皮行肺、胸膜、纵隔疾病自动活检的安全性和组织学诊断的准确性。方法 :在超声引导下使用带18G内槽式穿刺针的自动活检装置 ,对48例外周型肺占位病变 ,胸膜疾患和纵隔肿块的患者进行了96次穿刺活检诊断。结果 :穿刺成功率为100% ,组织学确诊阳性率为97 9% (47/48) ,穿刺后并发气胸1例占2 1 %。结论 :超声引导下自动活检是诊断外周型肺占位、胸膜疾病和纵隔肿块有效、安全及简便的方法 ,尤其是在肺外周型病变的鉴别诊断中 ,此方法可作为最佳选择。Objective:To determine the safety of ultrasound guided percutaneous automatic biopsy of pulˉmonary,pleural,and mediastinal disease,and the accuracy of histodiagnosis.Methods:Ninety_six consecutive biopsies were performed in48patients who suffered from peripheral pulmonary space occupying lesions,pleural disease,and mediastinal tumors.Under ultrasound guided,a18_guage core needle with automatic biopsy device was used.Results:The procedure of puncture was successful in100%patients.The correct histological diagnosis rate was97.9%(47/48).Pneumothorax was discovered in1patient(2.1%).Conclusion:Ultrasound guided percutaneous automatic biopsy is an effective,safe,and simple method for the diagnosis of peripheral pulmonary space occupying diseases and mediastinal tumors.The method may be the best choice for differential diagnosis of peripheral pulmonary diseases.
