摘 要:目的 研究静脉 (V)、基底膜 (BM)、雪旺细胞 (SC)和神经生长因子 (NGF)制备人工神经桥接 15mm兔坐骨神经缺损的作用。 方法 用日本大耳白兔 70只 ,分为 7组 (A、B、C、D、E、F、H) ,每组 10条坐骨神经。各组均修复坐骨神经 15mm缺损 ,以自体神经移植作为对照组 (A)。术后观察 3d、2 ,4,8和 12周肢体运动 ,肌电图动作电位波幅、潜伏期和传导速度 ,记录电生理结果。术后 12周取材 ,肉眼观察标本。比较近侧吻合口段 (a)、移植体中段 (b)、远侧吻合口段 (c)神经再生情况。随机半数组织行HE染色、免疫组化髓磷脂碱性蛋白 (MBP)染色。另半数组织做Cajar吡啶神经银染色。镜下观察神经再生情况、断端连接情况 ,并对各组再生神经束面积、有髓神经纤维密度、有髓神经纤维直径和轴突直径进行测量分析。 结果 在实验组中以H组 (V +BM +NGF +SC组 )修复情况最佳 ,上述各指标与自体神经移植组比较 ,差异无显著性意义 (P >0 .0 5 )。 结论V +BM +NGF +SC组修复神经缺损 ,效果最好 ,符合神经组织学结构 ,能达到自体神经移植的效果 ,很有可能成为周围神经缺损修复的一种新的方法。Objective To study the efficiency of artificial nerves made of vein (V), basement membrane (BM), Schwann cells (SC) and nerve growth factor (NGF) in bridging 1.5 mm sciatic nerve defects. Methods Seventy Japanese white rabbits were equally divided into 7 groups (Group A, B, C, D, E, F and H). Autogenous nerve grafting was used as control group (Group A). Sciatic nerve defects for 15 mm were repaired in all groups. The movement of limbs and amplitude of active potential, the latent period and the conduction velocity of electromusculography were observed at the 3rd day, the 1st, 2nd, 4th, 8th and 12th weeks after operation. Then, the electron biological results were recorded. After 12 weeks, samples were taken and examined by eyes. The neural regeneration at stumps A, B and C was compared. Random half of the samples were examined with HE staining and immunohistochemical staining of myelin basic protein (MBP); the other half with Cajar staining. Through microscope, the regeneration of nerves and the connection of stumps were observed. The area of regenerated nerve bundle, the density of myelinated nerves and the diameter of axon were statistically measured and analyzed. Results Group H (V+BM+NGF+SC) had the best regeneration in 7 groups. There was no significant difference upon parameters mentioned above compared with Group A (P>0.05). Conclusions Group H is the most effective in repairing long nerve defect and even can attain autograft, for it accords with the histological structure of the nerves. Therefore, it may provide a novel method for repairing the peripheral nerve defects.
