摘  要:目的 :探讨体外冲击波碎石术 (ESWL)致肾脏损伤区域的声像图特征、血流动力学改变及超声的诊断价值。材料和方法 :对 30例肾结石经ESWL治疗后出现肾脏损伤的患者进行超声检测 ,观察分析损伤区域的声像图特征、血流动力学的改变。结果 :ESWL致肾脏损伤的声像图特征为三种类型 :包膜下血肿 ,呈无回声区型 ;真性破裂 ,呈混合性回声 ;中央型破裂 ,呈低回声或无回声区型。损伤急性期 ,以上各型损伤区域内均未见血流信号 ;损伤恢复期在损伤区域可见血流信号。结论 :彩色多普勒超声对ESWL致肾脏损伤在急性期能够作出准确诊断 ,恢复期可观察损伤区域的血流动力学改变。Purpose:To investigate the echographic features of kidney and the changes of hemodynamics after extracorporeal shock wave lithotripsy(ESWL) using color Doppler ultrasound. Materials and Methods: 30 patients with renal stone and renal injury induced by ESWL were studied with ultrasound,and the echographic feature and the changes of hemodynamics in the injured area were analyzed. Results:There were three types of ultrasound findings for the ESWL induced-renal injury. Sub-membrane haematomaappeared as echo-free lesions; renal rupture displayed mixed echoes;and central rupture showed strong echo spot or no echo. At the acute stage after injury,there was no blood flow in the injured area but in recovery blood flow could be detected. Conclusion:Color Doppler ultrasound can provide accurate revealation for ESWL induced-renal injury and monitoring the changes of hemodynamics.
