北京市30～59岁人群代谢异常的聚集与2型糖尿病的关系分析

摘　　要：目的　分析中青年人代谢异常的聚集与 2型糖尿病 (DM )的关系。方法　横断面调查法调查北京市年龄3 0～ 5 9岁的中年人 60 2 9名 ,按空腹血糖≥ 7 0mmol/L的标准诊断DM。结果　各项代谢指标的年龄调整均数糖尿病组均较非糖尿病组高 (高密度脂蛋白胆固醇低 ) ;Logistic回归分析结果显示 ,有 4个代谢指标 ,包括体重指数 (BMI)、腰围、甘油三酯 (TG)、收缩压 (SBP) ,以及年龄与DM独立相关 ,OR (95 %CI)分别为 1 11(1 0 3～ 1 19) ,1 2 1(1 0 5～ 1 3 9) ,1 0 3 (1 0 2～ 1 0 4) ,1 2 3 (1 11～ 1 3 6)和 1 3 0 (1 14～ 1 49) ;调整年龄后 ,任 1、 2、 3、 4个上述代谢指标异常的聚集所对应的OR逐渐增大 ,各为 6 15～ 6 8、 8 5 8～ 12 69、 15 76～ 2 7 63和 3 5 5 2 ;几乎所有有显著意义的聚集组合均包括超重肥胖。结论　代谢异常聚集与DM密切相关 ,而人群中最危险的组合是包括肥胖的组合 ,控制代谢异常 。Objective To analyze the relationship between the aggregation of metabolic disorders and type 2 diabetes(DM)among the young and middle-aged people Methods Cross sectional study was carried out in a sample of 6029(30-59 year old)in Beijing urban area The diagnostic criteria for DM is fasting glucose≥7 0 mmol/L Results The age-adjusted means of metabolic factors in DM group were higher than its counterparts(HDL lower) Multivariate Logistic regression model showed there were 4 metabolic factors including BMI,waist,triglycerides(TG),and systolic blood pressure(SBP),plus variable age independently associated with the risk of DM,with OR(95% CI)being 1 11(1 03-1 19)、1 21(1 05-1 39)、1 03(1 02-1 04)、1 23(1 11-1 36)and 1 30(1 14-1 49),respectively After adjusted for age,the OR for any 1,2,3,or 4 clusters of above metabolic disorders increased dramatically,with the OR being 6 15-6 80、8 58-12 69、15 76-27 63 and 35 52,respectively Almost all of the statistically significant clusters included overweight and obesity Conclusion The cluster of metabolic disorders is closely related to the risk of diabetes,therefore,multi-factor especially overweight and obesity intervention is suggested for DM earlier control
