摘  要:目的 :探讨家兔胸部爆炸伤后早期血液流变学变化规律及其对肺脏等脏器功能的影响 .方法 :健康中国家兔随机分为对照组和伤后 5 ,30 ,6 0 ,180和 36 0min 5个时相点观察组 ,观察血液流变学及肺组织病理学动态变化 .结果 :家兔爆炸伤后早期全血粘度、血浆粘度、红细胞聚集指数、血浆纤维蛋白原含量及红细胞变形性均低于正常对照组 ,30min以后上述指标明显升高 ,180min后达峰值 ,与正常对照组相比有非常显著统计学意义 (P <0 .0 1) ,伤后 36 0min虽有所下降 ,但仍高于正常对照组 (P <0 .0 5 ) ;肺组织病理学变化与血液流变学变化基本一致 .结论 :家兔胸部爆炸伤后血液流变特性明显改变 ,血液粘滞性增加 ,从而加重微循环障碍导致脏器功能损伤 .AIM: To investigate the changes of hemorrheology and its significance for pulmonary function in rabbits following explosive chest injury. METHODS: Rabbits were subjected to explosive injury on their right chest wall. They were randomly divided into control group and injury group. The injury group was further divided into five groups on the basis of time points after the injury. Hemorrheologic characteristics were assayed by a viscometer R80. The histological changes in the lung were examined 1/12, 0.5, 1, 3 and 6 hours after the injury. RESULTS: After the explosive injury, the whole blood viscosity, plasma viscosity, the index of erythrocyte aggregation, plasma fibrinogen content and the index of erythrocyte deformability TK decreased compared with those of the control and 3 h after the explosive injury, it was the lowest. But they increased 30 min after the injury and reached the maximum 3 h after the injury ( P <0.01). The whole blood viscosity, the plasma viscosity, the index of erythrocyte aggregation and plasma fibrinogen content were lowered 6 h after injury but were still higher than those of the control ( P <0.05). Hemorheologic changes were correlated with the severity of the lung injury. CONCLUSION: The hemorrheologic characteristics have changed significantly after explosive chest injury, and the damage to the microcirculation may cause dysfunction of one or more organs, even multiple systemic organ failure.
