摘　　要：目的　探讨儿童持续性蛋白尿与血管紧张素转换酶基因多态性的关系。方法　用PCR方法检测了 186例儿童持续性蛋白尿患儿ACE基因多态性及ACE基因多态性与尿蛋白量、血尿、高血压、氮脂血症的关系。结果　持续性蛋白尿患儿ACE -DD基因频率为 0 .2 0 ,II基因频率为 0 .5 2 ,DI基因频率为 0 .2 8;持续性蛋白尿患儿ACE基因多态性与正常对照组存在明显差异 ;尿蛋白量与ACE基因多态性有关 ,大量蛋白尿多为ACE -II型 ;在紫癜性肾炎组血尿的发生以II型为主 ,氮脂血症的发生以DD型为主 ;肾病综合征组 ,高脂血症以II型为主。结论　儿童持续性蛋白尿与血管紧张素转换酶基因多态性有关。探讨儿童持续性蛋白尿与血管紧张素转换酶基因多态性的关系 ,对于早期发现和及时防治蛋白尿及其肾脏疾病有重要意义。Objective To investigate the association between persistent proteinuria in children with angiotensin converting enzyme gene polymorphism.Method We detected ACE gene polymorphism and the association between ACE gene polymorphism with urinary protein excreting weight ,hematuria,hypertension,azotemia using polymerase chain reaction in 186 children who suffered persistent proteinuria.Result The gene frequency of ACE-DD is 0.20,that of ACE-II is 0.52,that of ACE-DI is 0.28.There is significant difference in ACE gene polymorphism between the children who suffered persistent proteinuria with control subjects; urinary protein excreting weight is associated with ACE gene polymorphism. ACE genetype of mass-proteinuria patients is most ACE-II . The more occurrence of hematuria in purpuric-nephritis is ACE-II.The mone occurrence of azotemia in purparic-nephritisis ACE-DDThe more occurrence of is hyterlipemia in nephritic syndrome is ACE-II. Conclusion Persistent proteinuria in children is associated with angiotensin converting enzyme gene polymorphism.Investigating the association between persistent proteinuria in children with angiotensin converting enzyme gene polymorphism has important significance for detecting,preventing ,curing proteinuria and disease of kidney at earlier period.
