不同剂量丙种球蛋白治疗重症特发性血小板减少性紫癜近期疗效观察

摘　　要：目的　观察不同剂量丙种球蛋白治疗重症特发性血小板减少性紫癜 (ITP)的近期疗效。方法　 31例重症ITP患者分为两组 (17例、14例 ) ,分别用丙种球蛋白 4 0 0mg/ (kg·d)、5 0～ 6 0mg/ (kg·d)静滴 5d ,同时开始口服强的松 1mg/ (kg·d)。观察临床出血症状、血小板计数变化。结果　两组患者的近期有效率相近 ,治疗后血小板计数开始上升 ,升至≥ 5 0× 10 9/L ,达峰值的时间和血小板计数峰值、出血症状控制时间均相近 ,无显著性差异 (P >0 .0 5 )。结论　丙种球蛋白可迅速提高ITP患者血小板水平 ,短期内控制出血症状 ,中剂量丙种球蛋白的疗效与大剂量相近。Objective\ To evaluate the short-term therapeutic effects of different doses intravenous gammaglobulin in treating severe idiopathic thrombocytopenic purpura(ITP).Methods\ 31 patients with severe ITP were divided into two groups(17cases and 14 cases),respectively received high dose and medium dose intravenous gammaglobulin[400 mg/(kg·d) and 50～60 mg/(kg·d)]for five days and meanwhile started oral prednisone 1 mg/(kg·d).Observed the changes of blood platelet count(BPC)and various hemorrhagic symptoms. Results\ There was no significant difference in effective rate between two groups(P>0.05), so did in the time of BPC starting ascent, BPC ascending to ≥50×10 9/L, BPC ascending to peak and various hemorrhagic symptoms controlled, and so did in BPC peak. Conclusion\ Intravenous gammaglobulin can promptly increase the platelet count of patients with ITP and control the hemorrhagic symptoms in short term, and the therapeutic effect of medium dose gammaglobulin is quite similar to that of hige dosage.
