摘 要:目的 :利用超声心动图及有创压力监测仪与开胸的犬所制作实验性心包积液动物模型相结合 ,探讨超声心动图与心包积液量及心包腔内压之间的关系 ,分析急性心包填塞的成因 ,指导临床诊断及治疗。方法 :选用狗 (15条 )作为研究对象 ,构建心包积液动物模型 ,记录心包腔内所注入液体量 ,测定舒张末期 UCG (R)、(L )、 (A)心包积液液性暗区宽度 (mm) ,计算 (R+L +A) / 3的值 ,记录心包腔内压 ,依据线性相关及线性回归分析 ,得出它们之间的回归方程 ,预测心包腔内压。结果 :心包腔内压 (IPCP)与超声心动图 (R+L +A) / 3呈良好的线性对称正相关关系 ,线性方程为 :IPCP=1/ 9(R+L +A) - 41.2 2。结论 :急性心包积液并急性心包填塞时 ,(R+L +A) / 3及心包腔内压均随着心包积液量的增加呈现线性增高。因此 ,可以通过超声心动图来计算心包积液量并预测心包腔内压 。Objective:In our previous researches,we have found that PEV(Pericardiac effusion volume)can be estimated with echocardiography and PEV can be estimated based on section features of the heart measurements:the anterior aspect of the right ventricle wall(R),posterior aspect of the left ventricle wall (L)and the cardiac apex region(A).In the following researches,we tested the relationship among intrapericardial cavity pressure(IPCP),PEV and R,L,A,then deduced the formula to estimate the IPCP.Methods:Fifteen dogs‘ hydropericardium models were built up.The data about the volume of injection liquid,(R+L+A)/3 and IPCP were analysed to investigate the relationships among IPCP,PEV and R,L,A,then to deduce the formula to estimate the IPCP.Results:The results indicated the significant positive correlation of linear regression between IPCP and(R+L+A)/3.The regression formula is IPCP=(R+L+A)/9-41 22.Conclusions:1.The animal research revealed the facts that the IPCP was closely related to the(R+L+A)/3.With the calculation of (R+L+A)/3,the IPCP can be estimated according to the linear regression fomula,which will be very important to guide the timely treatment of acute pericardial tamponade.
