组织多普勒成像技术评价阵发性房颤心房收缩和电-机械时间的变化  

Measuring Atrial Contraction Time and Electromechanical Time by Tissue Doppler Imaging in Paroxysmal Atrial Fibrillation Patients

摘　　要：目的 :采用组织多普勒成像技术 (TDI) ,观察阵发性房颤 (PAF)患者左、右心房和房室环处心房收缩时间 (A)和电 -机械时间 (P- A)的变化。方法 :PAF组 61例 ,正常组 3 2例 ,取心尖四腔观 ,将 TDI取样容积分别置于室间隔房室环、左室侧壁二尖瓣环和右室侧壁三尖瓣环处 ,分别记录各点的的运动频谱 ,测量不同部位 P- A和 A值。结果 :与对照组比较 ,PAF组左、右心房径明显增大 ;各部位的 P- A、 A均显著延长 ;P- A、A与左、右心房径之间无明显相关 ,而与房颤持续时间、病史及年龄呈相关性 ,线性回归分析显示房颤持续时间是 P- A的主要影响因素。结论 :阵发性房颤患者心房收缩、电 -机械时间显著延长 ,且与左房、右房扩大无关 。Objective:To investigate the alteration of atrial contraction time and electromechanical time in different parts of atria among paroxysmal atrial fibrillation (PAF) patients by using tissue Doppler imaging (TDI) techniques.Methods:Two dimensional echocardiography and TDI data were acquired in 61 PAF patients with sinus rhythm and 32 normal subjects The TD images of various parts were obtained in apical 4 chamber view and the sample volume was placed at the interventricular septum of atrioventricular annulus, left ventricular lateral wall at mitral annulus and right ventricular lateral wall at tricuspid annulus,then each atrial contraction time which was measured from the onset of atrial contraction to cessation and electromechanical time which was meatured from beginning of P wave on surface ECG to the onset of atrial contraction were detected.Results:The dimension of left atrium(LA),right atrium(RA) in PAF group were much more greater than in those of controls.The P A interval (P A) and the duration of atrial contraction (A) were significantly longer in PAF patients than those in normal subjects.No relationship was present between P A and LA,RA,neither were A and LA.RA;P A,A were related to the duration of PAF episode,history of arrhythmia and age Linear regression analysis showed that the duration of PAF episode was the best variance value for P A prediction.Conclusions:Significant prolongation of atrial contraction and electromechanical time were found in PAF patients,and the variance values were not related to the dilation of left and right atria Duration of episode may promote the deterioration of arrhythmia.
