2型糖尿病高血压门诊患者药物治疗现状分析

摘　　要：目的 :了解目前门诊 2型糖尿病高血压患者用药现状 ,分析药物使用情况 ,为合理治疗提供依据。方法 :通过问卷调查 2 0 0 2年 1～ 6月上海某医院门诊 2型糖尿病高血压人群血糖、血压控制情况及降糖药、降压药服用情况 ,并对其用药种类、用药频度进行分析。结果 :2 0 0例 2型糖尿病高血压患者空腹血糖平均为 8.98mmol/L ,血压平均为 146 / 88mmHg。主要应用降糖药物依次为二甲双胍、格列齐特、格列吡嗪、阿卡波糖和胰岛素。降压药使用频度由高到低依次为 :珍菊降压片、硝苯地平、阿替洛尔、福辛普利、氯沙坦钾、吲达帕胺和复方降压片。β受体阻断药在有冠心病与无冠心病的患者中的使用无统计学差异 (P =0 .93)。结论 :目前 2型糖尿病高血压人群血糖及血压控制皆未达到推荐的标准。在用药方面 ,降糖药物选择较为合理 ,降压药物使用不甚妥当。还需加强对 2型糖尿病高血压患者关于用药知识及对血压控制的重视方面的宣传。Objective: To evaluate the medication patterns of hypoglycemic agents and antihypertension drugs as well as the quality of metabolic control and blood pressure in the outpatients of type 2 diabetes and hypertension.Mehtod: A questionnaire was used to ask the outpatients of type 2 diabetes and hypertension during Jan. to June 2002 in our hospital, noting down their blood glucose and blood pressure control as well as their drug prescribing patterns, and thereby evaluating the rationality of the drug use.Result: In a cohort of 200 patients with type 2 diabetes and hypertension, the average FBG was 8.98mmol/L, and the average blood pressure, 146/88mmHg. The mainly used hypoglycemic agents were: metformin, gliclazide, glipizide, acarbose and insulin, etc. The antihypertension agents were prescribed in the following descending order: Zhenju antihyptenersion tablets, nifidipine, metopropol, fosinopril, losartan, indapamide and compound antihypertension tablets. There was no statistic difference between the CHD patients and other patients in using β-adrenoceptor blockers.Conclusion: The glycemic and pressure control both remained unsatisfactory in view of the UKPDS recommendations. In the aspect of the drug use, the use of the hypoglycemic agents was proper while the use of antihypertension drugs was unsatisfied.
