摘 要:目的 :研究肠内营养 (EN)加免疫调节剂黄芪对创伤大鼠细胞免疫功能的影响。 方法 :将 4 0只SD大鼠随机分为四组 ,每组 10只 ,即正常对照组 (A)、创伤对照组 (B)、创伤后EN治疗组 (C)和EN加黄芪治疗组 (D)。双侧股骨离断及胃内硅胶管置入术建立创伤及肠内营养模型 ,早期 (创伤后 3~ 5h)给予EN及EN加黄芪连续治疗 6天 ,取血测定免疫指标T细胞表型CD4 + 、CD8+ 含量和血清白介素 2 (IL 2 )水平。 结果 :创伤后大鼠CD4 + T细胞、CD4 + /CD8+ 明显降低 (P <0 .0 1) ,血清IL 2水平亦明显下降 (P <0 .0 5 ) ;给予EN及EN加黄芪治疗后 ,CD4 + T细胞、CD4 + /CD8+ 值C、D组均高于B组 (P <0 .0 5 ) ,以D组最明显 (P <0 .0 1) ,血清IL 2水平D组明显上升 (与C组比 ,P<0 .0 5 )。 结论 :创伤后大鼠细胞免疫功能下降 ,早期EN可改善其细胞免疫功能 ,EN加黄芪治疗效果更佳 ,对创伤机体更具有保护作用。Objectives:To study the effects of enteral nutrition and Astragalus on cellular immune function of rats traumatized. Methods:Forty SD rats with bilateral femur fracture were randomly divided into four groups( n =10):group A,without trauma(normal control),group B(traumatic control), group C(enteral nutrition) and group D(enteral nutrition with Astragalus).The latter two groups received early enteral nutrition and enteral nutrition with Astragalus began after trauma and continued for 6 days. CD4 + cells and CD8 + cells were analysed by FACS,and concentration of serum IL 2 level was measured by ELISA. Results:CD4 + cell counts and CD4 +/CD8 + ratio were significantly decreased after trauma( P <0.01),and concentration of serum IL 2 was decreased( P <0.05).In groups C and D,CD4 + cell counts and CD4 +/CD8 + ratio were less decreased than that in group B,and the concentration of serum IL 2 in group D were less decreased( P <0.05) than that in group C. Conclusions:Bilateral femur fracture may impair the cellular immune function of rats.Early enteral nutrition and Astragalus can improve host immune response to trauma.
