摘 要:目的评价哌拉西林他唑巴坦(PIPC/TAZ)微泵持续输注治疗医院获得性肺炎的临床疗效和细菌学疗效。方法选择104例确诊为医院获得性肺炎的住院患者,采用前瞻开放性随机对照研究方法,试验组和对照组各52例。试验组给予PIPC/TAZ 3.0g/0.75g,q8h,微泵静脉持续输注3h;对照组给予PIPC/TAZ 3.0g/0.75g,q8h,静脉滴注30min。结果治疗后,试验组与对照组的总有效率分别为90.2%和72.5%,细菌清除率分别为87.5%和68.4%,2组间差异均具统计学意义(P<0.05);急性生理学及慢性健康状况Ⅱ系统评分均较治疗前改善,治疗后2组间差异有统计学意义(P<0.01);试验组平均疗程和住院时间均较对照组缩短,差异具有统计学意义(P<0.01)。结论 PIPC/TAZ微泵持续静脉输注3h治疗医院获得性肺炎,能显著提高临床疗效和细菌学疗效,缩短转归时间。AIM To evaluate the clinical and microbiological outcomes of piperacillin/tazobactam(PIPC/ TAZ)in the treatment of hospital-acquired pneumonia by continuous infusion in micro-pump.METHODS A total of 104 inpatients with hospital-acquired pneumonia were randomly divided into experimental group and control group,52 cases in each.A prospective randomized controlled clinical trial was conducted.The experimental group received PIPC/TAZ(3.0g/0.75g)every 8hby continuous administration using micro-pump for 3h.By comparison,the control group received PIPC/TAZ(3.0g/0.75g)every 8hby intravenous drip for 30 min.RESULTS After treatment,the total effective rates were90.2% and 72.5%,and the bacterial eradication rates were 87.5% and 68.4%respectively in the experimental group and in the control group,which showed statistically significant difference(P<0.05).Compared with the prior treatment,the acute physiology and chronic health evaluation scores had observably improved,which showed statistically significant difference(P<0.01).And,the average duration and hospitalization had also shortened in the experimental group compared with the control group,which showed significant difference(P<0.01).CONCLUSION It can improve the clinical therapy effects and bacteriological effect in treating hospital-acquired pneumonia by the continuous infusion of PIPC/TAZ for 3h.And also time can be shortened.
