产前二维超声筛选胎儿异常或畸形的探讨(附204例分析)

摘　　要：目的　利用二维超声产前筛查胎儿异常或畸形 ,提高人口出生质量。方法　收集了 1999年 1月至 2 0 0 1年 12月间在我院作B超筛选胎儿畸形的孕妇共 1780 3人 ,发现异常或畸形胎儿 2 0 4例 ,占 1.15 %。一般情况下 ,孕妇排尿后取仰卧位 ,在腹部扫查。特殊情况可改变体位 ;特殊脏器可以分时段或隔日重复检查 ,或结合其他辅助检查的特征辨别比较。结果　在 2 0 4例中 ,单发畸形 15 6例 ,占 76.47% ;多发畸形 48例 ,占 2 3.5 3%。头面部畸形发生率最高占 46.5 7% ;其次为泌尿生殖系统异常或畸形占 30 .88% ;伴发羊水过多者占 17.16% ;羊水过少者占 4.90 % ;其中绝大多数在本院或外院引产证实 ,极少数经过尸解。结论　二维超声可发现大多数的胎儿形态结构异常 ,使一些致死性的畸形及早终止妊娠 ,非致死性的异常或畸形有了选择的空间。在此基础上发现某些异常 。Objective Two-dimensional echocardiography is the basal for antenatal the fetus abnormality or malformation,is a current head to choose the method,clinical application extensive.On the child bearing quality of the boost body to make the indispensable effect,ought to get the extensive approbation of the medicine field.Method Collected 1999 year January to 2001 December the pregnant woman in our hospital making the two-dimensional echocardiography choosing the fetus malformation is totally 17803 people.Discover abnormality or malformation fetus 204 instances,occupy 1.15%.The general condition is down,and take to lie on the back after the pregnant woman urinate,and then sweep to check in the belly.Bit of special condition may convertibility body;Special and internal organs can timesharing zone or repeat survey every other day,or combination the other assist the survey's characteristic to distinguish and comparison.And combine with the induction of labor,autopsy's result to prove.Result Among the 204 instances,single happened malformation are 156 instances,occupy 76.47%;Several malformation happened together were 48 instances,occupy 23.53%.Head and face malformation's happen rate maximum to occupy 46.57%;The next is urinary and reproduction system abnormality or malformation occupy 30.88%;Companion happened by whose emniotic fluid excessive occupy 17.16%;Few emniotic fluid occupy 4.90%;Most of them were induction of labor prove in our or other hospitals,very few were go through by autopsy.Conclusion Two-dimensional echocardiography can discovering the most fetus's shape and construction abnormality as soon as possible,making some cause deathing malformation termination pregnancy,not cause deathing abnormality or malformation has selective space.Here some abnormalities which are discovered in basis,can adopt color dopplor flow imaging and three-dimensional echocardiography to do the further survey.
