妊娠糖尿病与基因突变关系的1:3病例对照研究

摘　　要：目的 :探讨妊娠糖尿病(GDM)与细胞毒性T淋巴细胞相关抗原 (CTLA4)基因及葡萄糖激酶 (GCK)基因之间的关系。方法:检测21例GDM患者及63例正常对照者CTLA4基因外显子1(CTLA4EX1)和启动子(CTLA4P)、GCK基因外显子6(GCK6)、外显子7(GCK7)的变异情况 ,用条件Logistic回归分析与GDM的关系。结果:单因素结果显示 ,CTLA4外显子1第49位A/G(OR=4.65695 %CI1.875～11.677)、启动子 -318位C/T(OR=10.17995%CI1.114～93.023)、GCK7变异 (OR=18.00095 %CI2.167～149.512)与GDM有统计学联系。结论:CTLA4外显子1第49位A/G变异和GCK7是GDM的独立危险因素 ,CTLA4启动子是GDM的危险因素 。Objective:To study the relationship of CTLA4gene and GCK gene with gestaˉtional diabetes mellitus.Methods:21GDM and63healthy women chosen form the same group that was paired-matched for age.Then using PCR-SSCP analysis to determine the mutations in the GCK exon6and exon7,PCR-RFLP analysis for CTLA4exon1and promoter.Results:The difference of the A/G transition polymorphism at49(exon1,codon17)of the CTLA4gene(OR=4.65695%CI1.875～11.677)and the C/T transition polymorphism at position-318(OR=10.17995%CI1.114～93.023)?GCK gene exon7(OR=18.00095%CI2.167～149.512)were statistically related to the development of GDM.Conclusion:The49A/G of CTLA4gene and GCK7are the independent risk factors of GDM.
