抗活化蛋白C对妊高征患者血栓前状态影响的研究

摘　　要：目的 :研究抗活化蛋白C(APCR)对妊高征患者血栓前状态的影响。方法 :采用ELISA法测定纤维蛋白肽A(FPA)、凝血酶原片段1 +2(F1 +2)、可溶性纤维蛋白单体复合物 (SFMC)、抗凝血酶 -Ⅲ抗原 (AT -Ⅲ :Ag)、蛋白C抗原 (PC :Ag)。采用SYSMEXCA -7000型凝血仪测定抗活化蛋白C(APCR)、抗凝血酶 -Ⅲ活性 (AT -Ⅲ :A)、蛋白C活性 (PC :A)、D -二聚体 (D -dimer)。结果 :与对照组比较 ,妊高征患者组F1 +2、FPA、SFMC、D -dimer水平均显著增高 (P<0.01),AT -Ⅲ、PC的活性和抗原水平均显著降低 (P<0.01) ,提示凝血系统显著活化 ,抗凝血系统功能显著减低。APCR阳性的妊高征患者组F1 +2、FPA、SFMC、D -dimer水平显著高于APCR正常的妊高征患者组 (P<0.01) ,AT -Ⅲ、PC的活性和抗原水平均显著降低 (P<0.01) ,提示APCR阳性的妊高征患者组血栓前状态较APCR正常的患者组更为显著。结论Objective:To Study the effect on prothrombotic state by activated protein C reˉsistance(APCR)in patients with pregnancy induced hypertension.Methods:Fibrin peptide A(FPA),prothrombin fragment 1+2 (F 1+2 ),soluble fibrin monomer complex(SFMC),anti-thrombin-Ⅲantigen(AT-Ⅲ:Ag)and protein C antigen(PC:Ag)were measured by enˉzyme-linked immunoadsorbent assay(ELISA).Activated protein C resistance,anti-thromˉbin-Ⅲactivation(AT-Ⅲ:A),protein C(PC:A)and D-dimer were measured using CA-7000blood coagulation analyzer.Results:The level of F 1+2 ,FPA,SFMC,D-dimer were sigˉnificantly higher and the level of AT-Ⅲ:Ag,AT-Ⅲ:A,PC:Ag,PC:A were obviously deˉcreased in pregnancy induced hypertension group than those in normal pregnant group and normal nonpregnant group,which showed that the coagulation system was significantly actiˉvated and the anti-coagulation system was obviously lower.The level of the parameters of coˉagulation system were obviously higher and the parameters of anticoagulation were significantly lower in the pregnancy induced hypertension group of APCR positive than those in the pregˉnancy induced hypertension group of APCR normal.Above changes of parameters showed that the prothrombotic state was more obvious in the pregnancy induced hypertension group of APCR positive than the pregnancy induced hypertension group.Conclusion:APCR can make prothrombotic state more serious in the pregnancy induced hypertension patients,so it is imˉportant to measure the APCR value and prevent the thrombosis.
