凝血-纤溶系统变化在妊高征凝血障碍机制中的作用

摘　　要：目的　探讨凝血 -纤溶系统变化在妊高征凝血障碍机制中的作用 ,为妊高征诱发DIC的预测提供有效诊断方法。方法　选择非孕组 40例 ,妊娠组 12 0例 ,妊高征组 5 0例 ,妊高征产后组 2 6例。采用双抗夹心酶联免疫吸附法 (ELISA)检测血浆凝血酶 -抗凝血酶复合物 (TAT)、D -二聚体 (D_dimer)含量　结果　 (1)正常早、中、晚孕TAT水平 ,与非孕组比较 ,差异有显著性 (P <0 0 5 ) ;中、重度妊高征组分别为 (15 4± 2 8) μg/L、 (2 0 5± 6 2 ) μg/L ,与晚孕组比较 ,差异有非常显著性 (P <0 0 1)。 (2 )正常早、中、晚孕D_dimer水平与非孕组比较 ,差异有非常显著性(P <0 0 1) ;中、重度妊高征组分别为 (1616 5± 14 8 1) μg/L、 (3 863 7± 3 42 4) μg/L ,与晚孕组比较 ,差异有非常显著性 (P <0 0 1) ;妊高征产后中、重度组较晚孕组明显增高 (P <0 0 5 )。结论　妊娠中晚期动态监测血浆TAT、D_dimer含量可做为DIC前状态敏感有效的辅助诊断。Objective To investigate the effects of changes in the coagulatiion and fibrinolysis system on the dysfunction of blood coagulation in patients with pregnancy induced hypertension (PIH) and to provide an effective method of diagnosis on the stimulation of the possibility of DIC induced by PIH. Methods Elisa method was adopted to test the amount of TAT and D_dimer in 40 non-pregnant women,120 normal pregnant,50 PIH,26 puerperas with PIH three-day after delivery. Results (1)There was a significant difference ( P <0.05) in TAT between the non-pregnant group and the pregnant in different phases.There was of significance difference ( P <0.01) in TAT between the moderate (15.4±2.8μg/L),severe (20.5±6.2)PIH patient and pregnant women in later trimester.(2)There was of significance difference( P <0.01)in D_dimer between the non-pregnant group and the pregnant in different phases.There was of singificance difference( P <0.01)in D_dimer between the moderate(1682.3±305.0μg/L),severe(3257.5±798.9μg/L)PIH patient and prenant women in late trimester.There was a significant increase( P <0.05) of D_dimer in patient with moderate and severe PIH after delivery than that in late trimester Conclusion TAT and D_dimer,as sensitive indicators,could contribute to the early diagnosis of DIC.
