摘 要:目的 :探讨螺旋CT三维 (3D)和多平面重建 (MPR)在髋、膝复杂关节骨折中的临床应用价值。材料与方法 :62例髋、膝关节骨折先行螺旋CT扫描 ,尔后作三维和多平面重建 ,注重观察骨折类型以及CT表现特征。结果 :62例中髋臼骨折 2 8例 (其中简单骨折 1 7例 ,复杂骨折 1 1例 ) ,胫骨平台骨折 34例 (其中累及外侧平台骨折 1 2例 ,内侧平台 8例 ,累及整个平台 1 4例 )通过 3D和MPR多方向的图像 ,直观、立体、清晰地显示了复杂的解剖特点和骨折的位置、形态 ,完善和补充了骨折分型。结论 :螺旋CT三维和多平面重建是轴位CT扫描有价值的补充手段 ,能为髋、膝复杂关节骨折的术前分类 ,治疗方案的选择及手术入路提供更多的客观依据 。Purpose:To study 3D and multiplanar reconstruction helical CT in evaluation of the complex injured hips and knees.Materials and Methods:62 patients with injured hips and knees underwent spiral CT scanning,then the source images were reformed with multiplanar reconstruction that focused on the type of fracture and characteristic of CT manifestation.Results:In 62 patients with 28 cases of acetabular fractures (17 cases belongs to simple fracture and 11 cases compex fracture) and 34 cases of tibial plateau fractures (12 cases belongs to lateral plateau fracture,8 cases medial plateau fracture and 14 cases total plateau fracture),3D and MPR images show the complex fractures from multiangular and three-dimensional,including their anatomy feature,shape and location,that improve and complete the classification of fractures.Conclusion:3D and MPR are supplementary means for axial CT scanning.We found that improved the classification of complex injured hips and knees before operation,the choice of the surgical intervention and hence better surgical planning and management.The method of 3D and MPR has higher valuation for clinic uses.
