培养基对肺炎链球菌抗生素敏感试验结果的影响

摘　　要：目的　研究培养基对肺炎链球菌体外抗生素敏感试验的影响程度。方法　同时应用普通血平皿和Mueller Hinton血平皿 ,对肺炎链球菌标准株ATCC4 96 19和临床分离的致病株以kirby Bauer琼脂扩散法进行 12种抗生素的敏感试验 ,用Etest法检测青霉素和红霉素的最低抑菌浓度。结果　复方SMZ在普通血平皿上的误判率最高 ,达 34.4 % ,错判程度也最严重 ,17.8%为敏感错判成耐药。居第二位的是氨苄西林 ,错判率17.9% ,其中 16 .1%为中介错判成耐药。青霉素、头孢曲松、头孢噻肟、四环素和氯霉素的错判率分别为3.6 %、3.6 %、1.8%、1.8%和 1.8% ,四种抗生素均以中介错判成敏感为主。红霉素、利福平、氧氟沙星和万古霉素在两种培养基上的药敏结果完全相同。头孢克罗在普通血平皿上的抑菌圈直径平均比药敏平皿上的大3.5cm ,两者差值超过 5cm的达 35 .9%。结论　用kirby Bauer琼脂扩散法对肺炎链球菌进行抗生素敏感试验必须在药敏专用培养基—MuellerObjective To investigate effective degree of different culture media on the susceptibility of 12 antibiotics against streptococcus pneumoniae.Methods Susceptibility test of 12 antibiotics against 56 streptococcus pneumoniae which were isolated clinically and streptococcus pneumoniae ATCC49619 were performed by Kirby\|Bauer method on both Columbia base medium with 5% sheep blood and Mueller\|Hinton medium with 5% sheep blood. Etest method was performed on Mueller\|Hinton medium with 5% sheep blood only.Results On Columbia blood agar, the activities of SMZco against 19 strains (34.4%) in vitro were wrongly determined (including 10 susceptible strains determined as resistant ones), followed by those of ampicillin against 10 strains (17.9%). The fault rates of penicillin, cepfatraxone, cefotaxime, chloromycetin and tetracycline were 3.6%, 3.6%, 1.8%, and 1.8% respectively, while the accuracy rates of vacomycine, ofloxacin, erythromycine and rifampin were 100% instead in susceptibility tests on Columbia base medium. Conclusion Susceptibility test of antibiotics against streptococcus pneumoniae by Kirby\|Bauer method should be performed on Mueller\|Hinton medium with 5% sheep blood instead of Columbia base medium with 5% sheep blood.
