腔内超声溶栓治疗肢体动脉闭塞性疾病的初步探讨

摘　　要：目的 :应用Acolysis血栓超声消融仪对下肢动脉硬化闭塞症 (ASO)和血栓闭塞性脉管炎 (TAO)进行治疗 ,评价其疗效。方法 :8例患者共 8条肢体均经血管造影和彩色多普勒超声确诊 ,其中ASO 5例 ,TAO 3例 ,病程在 15天至 3年。在DSA下用Acolysis血栓消融仪行腔内溶栓治疗。配合抗凝 ,祛聚 ,扩张血管等治疗。多普勒超声随访。结果 :10条闭塞的动静脉管腔均经一次治疗后再通 ,随访 6个月 ,1例TAO肢体因动脉再次闭塞而被截肢 ,其余 7例血管保持通畅 ,有效率达 87 5 %。结论 :血管内超声消融技术 ,可以有效地消融外周动静脉的陈旧性血栓和斑块 ,重新恢复闭塞血管的血流 ,从而能明显改善临床症状 。Objective:To use ultrasonic ablation to treat atherosclerosis obliterans(ASO),thromboangiitis obliterans(TAO) and evaluate its curative effect.Method:All of 8 cases(8 vessels) were checked by duplex ultrasound and angiography,5 cases of them were ASO,3 cases were TAO.The course of disease was 15 day to 3 years.Endovascular ultrasonic ablation was performed under Digital Subtraction Angiography(DSA).Thrombolytic,anticoagulant and vascular dilatation therapy followed the operation in all cases.All were followed up by duplex ultrasound.Results:7 occlusive arteries achieved re canalization after once therapy.Follow up after 6 month showed that 7 cases remained patent except 1 case TAO was accepted amputation.The effective rate reached 87 5%.Conclusion:Endovascular ultrasonic ablation can melt the old thrombus and atherosclerosis effectively and recovered the flow in peripheral vessel,therefore,improve the clinic symptom and save the limb.
