超声心动图在肺动脉闭锁合并室间隔缺损术前诊断中的作用

摘　　要：目的　探讨肺动脉闭锁合并室间隔缺损 (PA VSD)的超声心动图特点及诊断要点。方法回顾性分析 40例经手术证实为肺动脉闭锁合并室间隔缺损患者的超声心动图特点。结果　 40例患者中 ,有 3 7例在术前的超声心动图检查中得到确诊 ,另外 3例虽图像质量不佳 ,超声仍怀疑PA VSD ,无一例漏诊。结论　超声心动图对PA VSD可获得可靠诊断。在超声探测中注意观察肺动脉闭锁的水平、左右肺动脉有无融合、肺血管及右室发育状况、肺血来源以及合并其他心内畸形等 ,有助于正确指导外科手术的筛选及术式选择。ObjectiveTo improve the echocardiographic diagnostic accuracy of pulmonary atresia with ventricular septal defect (PA VSD).MethodsThe data of two dimensional and Doppler echocardiography in 40 patients with PA VSD confirmed by operation were analyzed retrospectively.Results Thirty seven patients were confirmed precise diagnosis by echocardiography before operation, 3 suspicious diagnosis of PA VSD though poor in imaging quality.ConclusionsPA VSD can be diagnosed accurately by echocardiography. Echocardiography diagnosis should include following main points:location of atresia,convergence of the left and right pulmonary arteries,development of right ventricle and pulmonary vessels,resource of pulmonary blood and other accompanied intracardiac abnormalities for instruction of operation choice.
