不同颞部除皱术式对秃发影响的比较

摘　　要：目的　比较不同除皱术式对秃发的影响 ,从而选择一种对秃发影响较小的手术方式 ,以利在临床中广泛采用。方法　将 1 1例患者分为A和B两组 ,分别在同一受术者的两侧术区采用不同的除皱术式。将颞浅筋膜浅层除皱术和颞浅筋膜深层除皱术分别与多层次除皱术比较 ,观察手术效果和对秃发的影响。结果　 1 1例患者均获得良好的手术疗效。A组和B组患者的左侧术区均出现范围大小不等的秃发、头皮结痂等现象。而A组和B组患者的右侧术区均无片状秃发和结痂现象的出现。结论　颞浅筋膜浅层除皱术、颞浅筋膜深层除皱术、多层次除皱术三者比较 ,经颞浅筋膜浅层分离的除皱术式对秃发的影响最大。Objective Compare 3 kinds of rhytidectomy procedures on the tendency and severity of alopecia formation and select a procedure that would show minimal tendency on its formation. Methods 11 patients were divided into two groups in which different procedures were performed alternatively on both sides in the same patient. The comparison was done according to the following arrangements. Group A, rhytidectomy with undermining above superfacial temporal fascia (left side) was compared with the pedicled multi-layer face lifting (right side). Group B, rhytidectomy with undermining under superfacial temporal fasica (left side) was compared with the pedicled multi-layer face lifting (right side). Results All of the 11 cases were satisfactied with the results. Different size and degree of patchy alopecia and scab formation were presented on the left side of A and B group with no such formation on the right sides of these two groups. Conclusion The operative procedure above superfacial temporal fascia gives the most obvious and severe patchy alopecia formation while in the pedicled multi-layer rhytidectomy it is minimal.
