断流术加食管下段胃底切除术治疗门静脉高压症上消化道出血的研究

摘　　要：目的　探讨断流术加食管下段胃底切除术治疗门静脉高压症上消化道出血的疗效。方法　采用择期断流术加食管下段胃底切除术治疗门静脉高压症上消化道出血 2 6例。患者均为肝炎后肝硬化伴有比较严重的食管胃底静脉曲张及 1次以上出血史。结果　全部病例均获随访 ,平均随访 5 2个月。随访中无食管曲张静脉复发出血 ,无手术近期死亡 ,效果满意。结论　断流术加食管下段胃底切除术能较好地为患者接受 ,复发出血率低 ,止血确切 。Objective To evaluate the effect of transabdominal gastric fundsectomy plus distal esophagectomy in patients with portal hypertension.Methods 26 cases with portal hypertension with a history of bleeding esophageal varix were treated by this procedure on elective basis.Results All cases were followed up for an average of 52 months.There was no recurrent hemorrhage and no mortality during the follow up.Conclusion This procedure is well tolerated by the patients with definite immediate effect and low recurrent bleeding rate.
