血清前白蛋白对实验性肝硬化大鼠肝脏储备功能的评价

摘　　要：目的　探讨前白蛋白 (PA)对实验性肝硬化大鼠肝脏储备功能的评价。方法　动态观察大鼠肝硬化诱导过程中PA的变化及其与常规肝功能、肝硬化程度和 MEGX之间的关系。结果　正常大鼠 PA平均为 2 70 .4± 3 2 .6m g/ L,肝硬化诱导的急性肝损害期、慢性肝损害期、轻 -中度肝硬化期及重度肝硬化期 PA分别为 12 5 .2± 2 4 .7mg/ L、168.7± 2 1.9mg/ L、13 1.4± 4 2 .3 mg/ L、10 2 .5± 3 6.6m g/ L,两两比较差异显著 (P<0 .0 1) ,尤其是急性肝损害及重度肝硬化时 PA下降更为明显。 PA降低与肝硬化程度一致。与 Alb比较 PA降低时间早且降低幅度明显。重度肝硬化期有腹水大鼠 PA为 74 .6± 19.5mg/ L,无腹水大鼠 PA为 117.7± 2 9.2 mg/ L,两者差异显著 (P<0 .0 1)。肝硬化诱导过程中 PA的变化与 MEGX之间存在显著正相关关系 (r=0 .67,p=0 .0 0 )。结论　Objective To investigate the serum value of prealbumin(PA) test for assessment of liver reserve function on experimental hepatocirrhosis in rats.Methods Ther serum value of PA was tested dynamically,and the correlation between PA and routine liver function test,the degree of hepatocirrhosis and MEGX were analyzed.Results The mean serum value of PA in normal rats was 270.4±32.6mg/L.The values of PA in acute phase of hepatic demage,chronic phase of hepatic demage,medium hepatocirrhosis and severe hepatocirrhosis were 125.2±24.7mg/L,168.7±21.9mg/L,131.4±42.3mg/L,102.5±36.6mg/L respectively.The differences between each others were significantly(P<0.01),the level pf PA was significantly decreased especially in acute phase of hepatic demage and severe hepatocirrhosis.The reduced degree of PA corresponded with the grade of cirrhotic degree of liver.The serum value of PA was decreased much more and earlier than that of Alb.The PA serum level of rats with ascides at severe hepatocirrhosis phase was 74.6±19.5mg/L,while that of rats without ascides was 117.7±29.2mg/L.The difference was significant(P<0.01).We had also found that there was a positive correlationship between the changes of PA and MEGX in the course of experimental inducement of hepatocirrhosis(r=0.67,p=0.00). Conclusion The serum value of PA can sensitively and exactly reflect the reserve ability and damage degree of liver function in experimental hepatocirrhosis in rats.
