摘  要:目的探讨断指再植后再植手指微循环损伤的特点及规律。方法动态检测断指再植后患者血清超氧化物歧化酶 (SOD) ,观察断指再植后缺血再灌注损伤的变化。用甲襞微循环测定仪动态观察再植手指甲襞毛细血管的扩张、渗出、出血、毁损、再生及微循环血管的血流状况。结果断指再植后SOD2周内持续低于正常水平 ,且于1~6h和24~72h形成两个低峰期。再植断指甲襞微循环恢复血液灌注后经历渗出、结构毁损和结构功能恢复三个时期。结论断指再植后缺血再灌注损伤至少在两周内持续存在 ,微循环损伤是再植手指缺血再灌注损伤最重要的部分。Objective To investigate the character and principle of microcirculation injury of reˉplanted severed fingers.Methods Dynamic measurement of SOD(Super_oxide dismutase)in serums,observaˉtion of changes of microcirculation re_perfusion injuries;Dynamic observation of blood state of replanted finˉgers and the changes of capillary loops of nail folds,including dilation,exudation,destruction,hemorrhage and regeneration of capillary.Results SOD is persistently under normal level within two weeks.and two negative peeks were formed during1~6hours and24~72hours.microcirculation of replanted fingers experienced three peˉriods.1.exudation period.2.structural lesion period.3.structural reestablish ment period.Conclusions Microcirˉculation re_perfusion injuries exist within two weeks.Microcirculation injuries are the most important parts of re_perfusion injuries of replanted fingers.
