摘  要:目的采用彩色多普勒超声(CDU)对腕部动脉检查 ,了解电损伤后血管性质的改变 ,以指导临床治疗。方法选择腕部电损伤患者10例 ,将前臂由近心端至远心端平均分为A、B、C三区 ,测尺、桡动脉血管内径 ,管壁厚度、内膜 ,彩色及脉冲多普勒血流信号 ,动脉PSV和阻力指数(RI)。结果与正常对侧比较 ,腕部电损伤后血管表现为 :1)二维超声图像 :B至C区尺、桡动脉管径缩小 ;管腔节段性扩张及“串珠样”改变 ;管壁变厚 ;血管内膜粗糙。C区损伤较B区明显。B区远端较近端管壁厚度、内膜变化更明显。A区与对侧正常肢体对应动脉表现基本相似。2)脉冲多普勒频谱表现 :PSV在B、C区平均在30cm/s,RI平均为0.7~0.9。3)彩色血流显像显示 :7例伤者腕部血管在C和B区血管透声性较差 ,彩色血流信号不完整 ,显示管壁毛刺状或充盈缺损 ,镶嵌色血流处峰值流速增快。结论CDU为电损伤后动脉的检查提供了一种非侵入性的、可靠诊断方法。CDU能较好地分析血管电损伤的程度 ,为临床治疗提供指导。Objective To observe the changes in wrist arteries damaged by electric burn to improve therapy.Methods Ten cases with wrist electric injury were enrolled in study.Their forearms from nearby heart to far from heart were divided averagely into three compartments:A,B,C.To examine arteries inˉner_diameter,vascular wall thickness,intima,color Doppler blood_stream signals,PSV and RI.Results Artery inner_diameter fromB to C got smaller,some got wider,and some arteries showed change of a string of beads.Vascular wall got thicker.Intima got rougher.Compartment C damage is more serious than that of compartment B.compartment A is almost normal.At compartment B and C:average PSV was about30cm/sec,RI is about0.7~0.9,and seven cases had poor echolucent quality and faintish color bloodstreamsignals.Conclusions Color Doppler ultrasound equipment provides a harmless reliable diagnostic means and shows the degree of the wrist arteries damage by electric burn to direct treatment better.
