摘 要:目的探讨一氧化氮 (NO)及肿瘤坏死因子α(TNF α)在妊娠高血压综合征及胎儿宫内发育迟缓发病中的作用及益气化瘀法提高胎盘血供的内在机制。方法采用L 精氨酸甲酯 (L NAME)于妊娠第 1 0d起皮下注射制作大鼠妊娠高血压综合征模型 ,黄芪丹参注射液及硝酸甘油干预 ,于妊娠第 1 8d用亚硝酸根还原酶法检测血浆NO水平 ,ELISA法检测血浆TNF α含量。结果模型组NO含量明显降低 (P <0 .0 1 ) ,TNF α水平升高 (P <0 .0 5 ) ,经黄芪丹参注射液治疗后 ,血浆NO水平升高 (P <0 .0 1 ) ,而阳性组TNF α水平不仅未降低 ,反而有所升高 ,NO含量稍有升高 ,但差异无统计学意义 ,并且NO与TNF α含量呈负相关。结论以益气化瘀立法的黄芪丹参注射液舒张血管、降低外周阻力、改善胎盘循环的作用可能是通过抑制TNF α、促进内皮细胞分泌NO而实现的。Objective To explore the effects of NO and tumor necrosis factor α(TNF α) on the develo pment of pre gnant hypertension syndrome(PHS) and fetal intrauterine growth retardation(FIGR) and the machenism of Qi boosting Blood quickening method(QBBQM) to increase p lace ntal blood supply. Methods Rat model was prepared by L Arginin e methyl ester(L NAME) subcutaneously injected to the pregnant animals on the 10th day of pregnacy.Then,Astragalus and Salvia Injection(ASI) and nitroglycer ine were used to cause intervening effect to the modeling animals.Following this,plasma NO level was me asured on the eighteenth day with silver nitrite reductase assay and TNF α le vel assaied with ELISA method.Results In model group, NO level was obvi ously lowered(P<0.01) and TNF α level elevated(P <0.01). After being treated with ASI, plasma NO level was raised(P <0.01). In positive controlling group, the TNF α level was shown raised so m ewhat and NO level was also slightly elevated but without significance. Further more, NO level was negatively correlated with TNF α content.Conc l usion The effects of blood vessel dilating,peripheral resistance decre asing and placental circulation improving of ASI,based on the pri nciple of Qi boosting and stasis transforming, are probably brought about due to inhibition of TNF α and due to promotion of NO secretion from endothelia l cells.
