摘　　要：目的 :探讨Holter检测中不同时段室性早搏及相关因素临床意义。方法 :选择 15 5例器质性心血管病患者 (病变组 )及 14 5例非器质性心血管病者 (非病变组 ) ,全部经动态心电图 (Holter)检查诊断室早LOWN分级 2级以上 ,比较两组室早高峰出现时间、Holter伴随的其它异常表现、患者年龄等相关因素。结果 :从时间段分析 ,白天室早多见于病变组 ,夜间室早多见于非病变组 (P均 <0 .0 5 ) ;但从临床分组来看 ,病变组与非病变组室早发生高峰均以白天多见。动态心电图伴随的房性心律失常、复杂室性早搏、心肌缺血等 ,病变组明显高于非病变组 (P <0 .0 0 1) ,尤其是心肌缺血 ,非病变组没有一例出现。结论 :室早的时段分布 ,随研究角度不同而不同 ,室早患者Holter伴随的异常表现意义更大。Objective: To analysis clinical significance of different time segments of ventricular premature beats and related factors by Holter monitor ECG. Methods: 155 patients with organic heart disease were selected as study group and 145 patients with non-organic heart disease as control group. All cases were diagnosed to have ventricular premature beats (≥Lown-Wolf Ⅱ)by Holter monitor ECG.The related factors,such as peak emergence time of ventricular premature beats,other abnormal appearance and the patients age were compared in two groups.Results:Analysis from the time segments,ventricular premature beats in daytime were most appeared in study group and in night-time most appeared in control group(P<0.05).But analysis from clinical grouping,peak emergence time of ventricular premature beats were most occurred in daytime in the two groups;atrial arrhythmia,complicated ventricular premature beats and myocardial ischemia of study group is significantly higher than that of control group(P<0.001),especially the myocardial ischemia ,there none occurred in control group. Conclusion: Since the different study analysis, the apperance of time segments of ventricular premature beats were difference. It has the most significance in the patients who have ventricular premature beats accomany abnormal apperance in Holter monitor ECG.
