医院社会效益及质量竞争对其影响的理论分析

摘　　要：目的 借用产业经济学中的概念——“消费者剩余”和“生产者剩余”,引进“患者剩余”和“医院服务剩余”的概念,并提出“医疗质量价格”等概念。运用这些概念对医院社会效益和经济效益进行衡量和分析,并在此基础上分析医院质量竞争的动因,以及质量竞争对医院社会效益的影响。方法采用新概念引入与分析、数学建模以及积分运算等方法。结果 提出了衡量分析医院社会效益的新思路和新方法,明确了医院质量竞争对医院社会效益的影响。结论“患者剩余”等概念对于分析医院的社会效益具有重要意义,并易于数学运算。通过对医疗质量竞争对医院社会效益影响的分析,提出质量管理与成本管理并举、加强医疗服务流程的质量管理、把医院成本节约体现到患者剩余中、优化院长业绩评估体系等建议,对医疗质量竞争对医院社会效益的影响加以控制和优化。Objective The concepts of 'patient's surplus' and 'hospital's senice surplus' were introduced from such concepts of industrial economics as consumer's surplus and producer's surplus , and the concept of 'price of medical quality was presented. These concepts were used to measure and analyze the social and economic benefits of the hospital, and on the basis of this, the agent of quality competition of the hospital and the effect of quality competition on the social benefits of the hospital. Methods Such methods as the introduction of and analysis by new concepts, mathematical modeling and infinitesimal calculus were used. Results New thoughts and measures for measuring and analyzing the social benefits of the hospital were put forward and the effect of quality competition on the social benefits of the hospital was specified. Conclusion Concepts like 'patient's surplus' are of great significance to the analysis of the social benefits of the hospital and are easy to operate mathematically. By analyzing the effect of medical quality competition on the social benefits of the hospital, some suggestions have been put forward, including simultaneous promotion of quality control and cost control, enhancement of the quality control of the medical service process, embodiment of the hospital's cost savings in patient's surplus, optimization of the assessment framework of the hospital director's performance, and control and optimization of the effect of medical quality competition on the social benefits of the hospital.
