经皮冠状动脉切割球囊血管成形术前后血小板活性变化及临床意义

摘　　要：目的　研究经皮冠状动脉切割球囊成形术 (CBA)前后血小板活性变化及其临床意义。方法　将患者随机分为切割球囊组和普通球囊 (POBA)组 ,由冠状静脉窦采血 ,采用生物活性法测定血小板衍生生长因子 (PDGF) ,采用酶联免疫标测法 (ELISA)检测 6-酮 -前列腺素F1а(6-酮 -PGF1a)浓度。结果　CBA组与POBA组比较 ,PDGF及 6-酮 -PGF1a值在PTCA术后均存在显著性差异 (P <0 0 5 )。且PDGF呈时间依赖性上升 ,3 0min仍维持在高水平。 6-酮 -PGF1a值术后两组均呈一过性降低 ,切割球囊组术后10min恢复至术前水平 ,而普通球囊组恢复时间延长至术后 3 0min。结论　切割球囊是用于PTCA的新型球囊 ,可减轻血管壁损伤 ,降低血小板活化水平 ,可能是降低再狭窄的重要原因之一。Objective To study the platelet activity changes in patients before and after percutaneous coronary cutting balloon angioplasty (CBA) and plain old balloon angioplasty (POBA) as well as its clinical significance.Methods Blood was obtained from the coronary sinus. Bioassay method used for assessment of PDGF. The plasma 6-keto-PGF 1a level was determined using ELISA.Results The amount of PDGF and 6-keto-PGF 1a were significantly different after PTCA between CBA group and POBA group (P<0 05). Moreover,the amount of PDGF was higher after PTCA than that before PTCA elevation of PDGF was dependent upon the postoperation time,and steadily increasing at 30 minutes. The amount of 6-keto-PGF 1a was lower after PTCA than that before PTCA, recovering to the normal at 10 minutes in CAB group, while at 30 minutes in POBA group.Conclusions The cutting balloon is a new device for coronary angioplasty, which enter into combination with resection the plaque and dilation of blood vessel, is believed to minimize arterial wall trauma, the neoproliferative response, and may be one of the reasons of reducing subsequent restenosis.
