参麦注射液对高血压左心舒张功能不全的影响

摘　　要：目的:观察参麦注射液对高血压左心舒张功能不全的影响。方法:选择原发性高血压中医辨证属气阴两虚证患者60例,随机分为治疗组与对照组,对照组服用络活喜、培那普利、倍他乐克,治疗组在上述药物基础上,静滴5％葡萄糖注射液加参麦注射液40mL/d,疗程15d,观察两组的降压疗效和症状疗效,并采用超声心动图测定治疗前后左室舒张功能参数。结果:治疗组与对照组降压疗效大致相似;治疗组症状疗效优于对照组;两组治疗前后左室重量变化均无显著差异;舒张早期与舒张晚期峰值流速的比值(E/A)以及舒张早期流速的减速时间(DT)、等容舒张期(IRT)等指标,对照组治疗前后差异无显著性,治疗组治疗前后差异非常显著,组间差异亦非常显著。结论:参麦注射液可改善高血压患者气阴两虚症状,可改善高血压早期左室舒张功能不全,该作用不依赖于左室肥厚的逆转而实现;提示其作用机理可能与中医证存在相关性;在高血压治疗早期加入参麦注射液疗效可能更好。Objective: To observe the effect of Shenmai injection dealing with insufficient relaxation of left ventricle caused by hypertension. Method : Sixty patients whose presenting syndrone comespond with deficiency of both Qi and Yin according to traditional Chinese medicine theory while who suffered from essential hypertension was chosen for observation. These patients were assigned randomly either to the treated group or to the control group. The patients in the control group take Amlodipine, Lotensin and Bateloc. Aside from the aboved oral medicine, the patients in the treated group was given by IV infusion Shenmai injection 40mL in 5% glucose 500mL, Qd. 15 days constituting a single therapeutic course. Finally, we compare the hypertensive effect and the symptom between two groups, while measure the relaxing parameter of left ventricle before and after treatment, by ultrasonic cardiograme. Result : The hypertensive effects between two groups are indistinguishable. The symptomatic improvement in the treated group is superior to the control group. The weight variation of left ventricle between before and after treatment both in two groups were insignifcant. The targets, such as largest blood flow rate odds between protodiastole and telediastole, deceleration time in protodiastole, isovolumic relaxation time, etc. are insignificant between before and after treatment in the control group and are opposite in the treated group and between two groups. Conclusion: Shenmain injections can not only improve the patient's symptoms with hypertension, but also the left ventride's insufficcent relasation. It can't change left ventricular hypertrophy. The test suggests that the function can be correlated with the syndrome in traditional Chinese medicine and that Shenmai injection used in the earlier hypertension will be better.
