肾盂输尿管镜下置管扩张法治疗复杂性尿道狭窄(附26例报告)

摘　　要：目的 :探讨肾盂输尿管镜下置管扩张法治疗复杂性尿道狭窄的临床疗效。方法 :骶管麻醉后用F8/ 9.8肾盂输尿管镜检查尿道狭窄情况 ,直视下将 F- 3号输尿管导管通过狭窄段置入膀胱 ,再利用镜体本身及液压灌注泵的扩张作用 ,进一步扩大了狭窄段管腔 ,置入 3根 F- 3或 F- 5号输尿管导管 ,以后每周给予递增不同型号的导尿管至 F- 2 2号 ,扩张狭窄段引流膀胱尿液 ,共治疗 2 6例。结果 :全部病例都在门诊治疗 ,6～ 8周后均可扩张至 F- 2 2号 ,拔管后自行排尿通畅 ,2例需定期做尿道扩张 ,随访 6个月～ 3年无复发。结论 :肾盂输尿管镜下置管扩张法治疗复杂性尿道狭窄 ,具有操作简单灵活、直视下进行 ,对病人打击小 ,损伤轻 ,恢复快 ,不需住院 ,费用低 ,疗效确切 ,可反复操作 。Objective:To explore the curative effects of intubating dilation under pelviureteric speculum in treating complex urethrostenosis.Methods:F8/9 8 pelviureteric speculum was used to check the urethrostenosis after the sacral canal was anesthetized.F 3 urinary catheter was placed into bladder through the stenosed segment under direct vision.F 3 or F 5 catheter was used to further dilate the stenosed lumen with the help of the speculum and hydraulic perfusion pump.More different types of catheters were used in the following weeks until the F 22 catheter was intubed.This method was used in 26 cases to dilate the stenosed segment.Results:After 6～8 weeks of treatment in the out patient department,unobstructed urination after decannulation occurred in all cases.Only two cases needed periodic urinary catheter dilation because of white cicatrices.No recurrence was found out in the follow up surveys for 6 months to 3 years.Conclusion:Intubating dilation of pelviureteric speculum can be performed easily and effectively under direct vision.Patients can recover quickly with low costs.It has proven to be a new effective method for treating complex urethrostenosis.
