肾移植术后环孢素A所致药物性牙龈病的治疗

摘　　要：目的　探讨肾移植术后环孢素A所致药物性牙龈病 (GDM)的治疗。方法　 4 8例肾移植后发生GDM的患者随机分为 3组 ,分别采用手术治疗、体外培育牛黄糊剂局部治疗及基础治疗 ,观察治疗 1个月后牙龈肿大、疼痛、出血、龈沟出血指数、龈袋深度的变化。结果　总的治疗效果 ,手术治疗组和体外培育牛黄糊剂局部治疗组的效果相仿 ,但明显优于基础治疗者。结论　体外培育牛黄糊剂治疗GDM疗效较好 ,不良反应小 。Objective To evaluate the relation gingival diseases modified (GDM) by cyclosporin (CsA) and community periodontal index (CPI), local dysexcitation (LDE), CsA density of blood, saliva, gingival constitution and therapeutics method.Methods CPI and LDE of 234 patients were checked up before taking CsA 0.5 , 1 and 1.5 year(s) after administration of CsA, incidence of GDM and CsA density of blood, saliva and gingival constitution were assayed. 48 patients with GDM were selected by unilateral blind random method, of whom 16 cases were treated with gingivectomy as the operation experimental group, of whom 15 cases were treated with calculus bovis cultured in vitro (CVP) as the medication experimental group, as 17 cases served as the control group. Clinical examinations were evaluated by periodontitis indexes. Results The incidence of GDM by CsA was the highest after administration of CsA for 1.5 years. Incidence, severity and locus of GDM and CPI, LDE, CsA density of blood, saliva and gingival constitution were related to each other. The periodontitis indexes (GI, PD) were decreased as compared with the control ( P < 0.05 ). The clinical experiments showed that the rate of effectiveness and of clinical cure and of total effective of medication and operation were equal ( P > 0.05 ).Conclusion Decreased CPI and early treatment are utility of prevention and cure for GDM. The clinical effect of CVP and operation is equally good.
