冠状动脉内超声检查在支架内再狭窄研究中的临床应用

摘  要:目的 :比较常规冠脉造影、冠脉造影计算机定量分析和冠脉内超声 ,探讨冠脉内超声在支架内再狭窄研究中的应用。方法 :对 4 1例病人的 4 5处病变置入支架治疗 ,术前、术后采用冠脉造影和冠脉内超声系列检查。结果 :术前、术后即刻以及术后 3个月、6个月冠脉造影病变狭窄程度分别是 87.4± 11.3% ,8± 9.2 % ,4 9.8±10 .7%和 5 7.2± 12 .3% ;QCA示狭窄程度分别为 6 1.4± 16 .0 % ,3.3± 13.3% ,2 4 .8± 2 0 .6 %和 2 8.9±18.8% ,两种方法得出的结果有显著性差异。冠脉内超声示术后即刻以及术后 3个月、6个月的支架横断面积从 8.2 5± 1.83mm2到 95 8± 2 .5 9mm2 (P <0 .0 0 1)和 10 .4 6± 2 .6 9mm2 (P <0 .0 1) ,支架横断面积进行性扩大 ,前后比较在统计学上有显著性差异。结论 :ICUS是研究支架内再狭窄的一个最直接、客观的方法 ,安全、易操作。Objective:The purpose of this study is to investigate the outcome of intracoronary ultrasound(ICUS) for in-stent restenosis research. Methods:41 patients with coronary artery disease were studied. We compare the outcome of angiography, quantitative coronary angiography (QCA) and ICUS.Results:Successful radius stent deployment was achieved in 45 lesions of 41 patients with coronary artery disease. Angiography and ICUS were performed at baseline, after stent implantation and at 3-,6-month follow-up. Lesion stenosis of angiography is 87.4±11.3%, 8±9.2%, 49.8±10.7% and 57.2±12.3% respectively, there are statistical differences with lesion stenosis of QCA: 61.4±16.0%, 3.3±13.3%, 24.8±20.6% and 28.9±18.8% respectively. Radius stent CSA increased gradually from after implantation to 3-month and 6-month follow-up (8.25±1.83, 9.58±2.59 and 10.46±2.69mm 2, P <0.01 respectively).Conclusions:These findings indicated that ICUS is effective for in-stent restcnosis resaerch.
