霉酚酸酯联合中药治疗难治性原发性肾病综合征

摘　　要：目的 :观察霉酚酸酯 (MMF)联合中药和小剂量糖皮质激素治疗难治性原发性肾病综合征 (NS)的临床疗效及安全性好。方法 :用MMF联合中药和小剂量糖皮质激素治疗 19例难治性原发性肾病综合征患者 6个月 ,观察疗效和副反应。结果 :完全缓解 4 2 .1% ,部分缓解 31.6 % ,总有效率为 73.7%。全部病例 2 4h尿蛋白定量、血清纤维蛋白原、总胆固醇明显下降 ,血清白蛋白明显上升 (P <0 .0 1)。以微小病变型、系膜增生型、膜性肾病型Ⅰ期病例的疗效更为明显。除 1例肝功能轻度异常、1例胃肠道反应外 ,来发现其它明显副作用。结论 :霉酚酸酯联合中药和小剂量糖皮质激素治疗难治性原发性肾病综合征临床疗效好而快 ,副作用小 ,有临床推广应用价值。但远期疗效还有待进一步研究和探索。Objective: To investigate the efficacy and safety of mycophenolate mofetil (MMF) and chinese drugs on treating refractory primary nephrotic syndrome.Methods: 19 patients ruth refractory primary nephrotic syndrome were treated by MMF combined with chinese drugs and prednisone. Follow-up interview was eonducted regularly. Results: The rate of complete clinical remission was 42.1% and the total effective rate was 73.7%.The urine protein the serum fibrinogen(FIB), and the serum total cholesterol(TCH) of thc patients significantly decreased but on the other hand the sertun albumin distinctly inereased ( P <0.01 or P <0.05) Renal function remain stable during the treatment. All patients with MCD and MsPGN achieved clmiealremission, and six of the eight cases obtained complete cFmical remission two cases obtained complete clinical remission, two cases achieved clinical remission and two cases had no efficacy among six patients with MN. During the treatment, side effects were tolerable .Conclusions: The method of MMF and chinese drugs on treating refractory primary nephrotic syndrome is effectiveand safe.
