宫内节育器宫腔形Cu300、TCu220C、TCu380A放置10年的临床比较性研究

摘　　要：目的　了解宫腔形宫内节育器 (Cu30 0 )及TCu2 2 0C、TCu380A3种宫内节育器 (IUD)放置1 0年的临床效果。方法　全国 7个IUD临床应用中心 ,对 2 699例随机放置Cu30 0、TCu2 2 0C和TCu380A的农村妇女 ,共观察 1 0年。结果　使用 1 0年时 ,Cu30 0、TCu2 2 0C和TCu380A的带器妊娠率 ,分别为 2 56、4 82和 4 0 1 / 1 0 0妇女 ,Cu30 0明显低于其他两种IUD(P <0 0 5)。脱落率分别为 4 54、5 46和 9 68/ 1 0 0妇女 ,TCu380A显著高于其他两种IUD(P <0 0 0 1 )。因出血、疼痛取出率分别为6 71、5 58和 6 80 / 1 0 0妇女 ,3种IUD比较 ,差异无显著意义 (P >0 0 5)。使用 1 0年时 ,3种IUD的续用率分别为 74 86、66 2 2和 63 56/ 1 0 0妇女 ,以Cu30 0续用率最高 ,TCu380A最低 (P <0 0 1 )。结论　Cu30 0、TCu2 2 0C和TCu380A的避孕效果好 。Objective To observe the clinical effects of uterine cavity shaped device (UCD )Cu300,TCu220C and TCu380A in ten years Methods Two thousand, six hundred and ninety nine rural parous women were randomized into three groups UCD Cu300 group had 899 cases, TCu220C group and TCu380A group each had 900 cases Follow up visits were performed at 3, 6, 12 months and 2, 3, 5, 10 years after insertion Results The cumulative pregnancy rates in the 10th year for UCD Cu300, TCu220C and TCu380A were 2 56, 4 82, and 4 01 per 100 women, respectively UCD Cu300 group was significantly lower than other two groups ( P <0 05) The expulsion rates were 4 54, 5 46 and 9 68 per 100 women, respectively TCu380A was significantly higher than UCD Cu300 and TCu220C ( P <0 001) The rates of removal for bleeding and pain for UCDCu300, TCu220C and TCu380A were 6 71, 5 58 and 6 80 per 100 women, respectively, no statistical difference was shown The cumulative continuation rates were 74 86, 66 22 and 63 56 per 100 women at the 10 th year, UCD Cu300 was the highest, TCu380A was the lowest ( P <0 01) Conclusions UCD Cu300, TCu220C and TCu380A are effective for long term contraception They are worthy of wide spreading in rural areas
