摘  要:目的探讨畸变产物耳声发射(distortion product otoacoustic emissions,DPOAE)对精神性耳聋的诊断应用。方法采用ILO96耳动态分析仪对精神性耳聋进行DPOAE检查与分析。结果本组精神性耳聋有10例18耳(占90%)的DPOAE结果显示正常,有1例2耳(占10%)的DPOAE在某个频率不能检出,部分频率的反应幅值降低,显示该例存在轻度耳蜗病变。表明精神性耳聋虽系非器质性病变,但不能排除其发病时可能存在一定程度的器质性病变。听性脑干反应(auditory brainstem response,ABR)检测本组精神性耳聋的反应阈值,证实DPOAE的检查结果客观、可靠。结论 DPOAE作为一种常规的听力学检查方法,可对精神性耳聋作出及时、有效、客观、可靠的检查与诊断。Purpose To explore the application of distortion product otoacoustic emissions (DPOAE) in the diagnosis of mental deafness. Methods Detection of DPOAE in mental deafness was performed with ILO96 Otodynamic Analyzer. Results The results of detection of DPOAE showed that 18 ears (90%) in 10 patients with mental deafness were normal in this study. DPOAE at a certain frequency was not able to be detected in 2 ears (10%) of one patient and amplitudes of the partial frequency decreased, which indicated existence of mild cochlear lesion. It showed that the possible existence of organic lesion to a certain extent at the onset of mental deafness could not be excluded, although these diseases were nonorganic lesions. The reactive threshold value determined by auditory brainstem response (ABR) in these patients with mental deafness confirmed mat the results of DPOAE were objective and reliable. Conclusion DPOAE can be used as a routine examination method of audiology. It gives a prompt, effective, objective, and reliable examination and diagnosis for the mental deafness.
