摘  要:目的:分析糖尿病是否影响手指离断再植治疗效果.方法:对210例513断指依据病史分为糖尿病组(100例离断228指)和非糖尿病组(110例离断285指);依据糖化血红蛋白检测水平将糖尿病组分为分为血糖控制良好组和血糖控制不良组,两组离断指分别为126指和102指.所有患者按致伤性质采取相应的治疗方法,根据临床指征统计其成活率.术后对离断再植成活指定期进行功能评定,将评定等级分为优、良、差,并统计优良率.结果:513个断指再植后存活390指,存活率达76.0%.血糖控制良好组和非糖尿病组断指再植术后成活率分别为81.8%和87.0%,高于血糖控制不良组术后成活率38.2%,差异具有统计学意义(P<0.05).血糖控制良好组术后成活率与非糖尿组成活率比较、血糖控制不良组与血糖控制良好组术后功能评定优良率比较、血糖控制良好组与非糖尿组术后功能评定比较,差异无统计学意义(P>0.05).结论:糖尿病对手指离断再植术具有较大影响,术后控制糖化血红蛋白水平是治疗此类疾病的关键.Objective:To analyses whether diabetes affects the treatment outcome of severed finger replantation. Methods:210 patients(513 amputated fingers)according to their medical history were divided into diabetic group(100 cases with 228 amputated fingers)and non-diabetic group(110 cases with 285 amputated fingers). According to the patients' glycosylated hemoglobin levels, the diabetic group was subdivided into good blood glucose control group(126 amputated fingers)and poor blood glucose control group(102 amputated fingers). All patients received appropriate treatment according to the nature of the injury, and the survival rate after amputated finger replantation was statistically analyzed according to the clinical indications. The survived fingers also underwent functional assessment on a regular basis, the rating was divided into excellent, good, bad, and the rate of excellent and good was statistically analyzed. Results:In 513 amputated fingers, 390 fingers were survived after finger replantation, the survival rate was 76.0%. The survival rate of the good blood glucose control group(81.8%)and the non-diabetic group patients(87.0%)was higher than the poor blood glucose control group(38.2%), the difference was statistically significant(P<0.05). However, the survival rate between the good blood glucose control group and the non-diabetic group, the excellent and good rate of functional assessment between the poor glucose control group and the good blood glucose control group, the postoperative functional assessment between the good blood glucose control group and the non-diabetic group was not statistically significance difference(P>0.05). Conclusion:Diabetes has a great impact on the success of finger replantation surgery, so the control of glycosylated hemoglobin level after surgery is the key to treat this kind of diseases.
