静脉营养疗法治疗COPD并营养不良的临床研究

摘　　要：目的　观察静脉营养支持治疗 COPD合并营养不良患者的临床疗效。方法　将 5 8例 COPD合并营养不良患者随机分成两组 ,治疗组 2 8例在正常饮食基础上予清蛋白乳化脂肪液氨基酸液等静脉营养支持 ,对照组 30例除正常要素饮食之外 ,不予静脉营养治疗。两组均予抗感染、吸氧、化痰、解痉等治疗 ,疗程4周。结果　与对照组比较 ,治疗组人体测量增加、肺功能改善、血清清蛋白升高 ,有显著差异 ( P <0 .0 5 ) ,总淋巴细胞计数升高 ( P <0 .0 5 ) ,但细胞免疫未见改善。结论　静脉营养支持治疗可以改善 COPD合并营养不良患者的营养状态 ,改善肺功能 ,提高免疫机能。Objective To study the effect of intravenous nutrition in the treatment of chronic obstructive pulmonary diseases (COPD) accompanied with malnutrition.Methods 58 patients with COPD accompanied with malnutrition were divided into two groups randomly. 28 patients were given normal food, combined with albumin, chyle fat and amino acid infusion; the others were given normal food only. Two groups were received antibiotic, oxygen, expectorant and antispasmodic for 4 weeks.Results In tested group, the patient's body weight increased, the lung function, serum albumin and total count of lymphocyte also increased, compared with controls group (P<0.05). But the patient's cellular immunity did not change.Conclusions Intravenous nutrition can improve the nutritional condition, the lung function and increase the body immunity in patients with COPD accompanied with malnutrition.
