纤维支气管镜在儿科呼吸系统疾病中的临床应用

摘　　要：目的 :研究纤维支气管镜 (纤支镜 )对小儿呼吸道疾病的诊断、治疗价值及其安全性。方法 :对该院 2 0 0 0年 1月～ 2 0 0 1年 9月应用纤支镜检查的 11例共 2 3例次患呼吸系统疾病的小儿进行回顾性分析。观察纤支镜检查的临床效果。结果 :男性 8人 ,女性 3人 ,年龄最小 2 9d ,最大 7岁。肺不张 5例 ,经纤支镜治疗、调整抗生素应用及气道内介入治疗等综合措施处理后 ,均获得肺复张。难治性肺炎 3例、咯血查因 2例经纤支镜检查明确病因后及时调整治疗方案 ,取得良好效果。 1例吸气性喘鸣患儿经纤支镜检查确诊为声门下重度狭窄。不良反应包括低氧血症 7例 ,室上性心动过速 1例 ,一过性呼吸抑制 1例。结论 :纤支镜在儿科呼吸系统疾病的诊断和治疗上具有重要价值 ,但应掌握指征 ,合理应用 ,以减少并发症的发生。Objective: To study the diagnostic, treatment value and the safety of flexible fiberoptic bronchoscope examination applied in pediatric patients with respiratory diseases.Methods: We examined 11 children in all for 23 times ranging in age from 29 clays to 7 years during Jan. 2000 to Sep. 2001 using children fibrobrochoscope and adult fibrobrochoscope.Results:Patients included 8 males and 3 females who's indications were atelectasis for 5 cases, all of them obtained lung re-expansion after washing, suction, changing antibiotic according to the culture reports and interventional therapy. The other patients with persistent pneumonia for 3 cases,hemoptysis for 2 cases and inspiratory stridor for 1 case acquired diagnosis confirmed. Complications included transient hypoxymia for 7 cases, supraventricular arhythmia for 1 case and transient respiratory depression for 1 case.Conclusion: Flexible fiberoptic bronchoscope serves as an important aid in the diagnosis and therapy of disorders of the respiratory tract in small infants and children and is a safe procedure with enough caution.
