Objective To explore the feasibility of cryopreserved aortic homograft in replacement of the inferior vena cava. Methods A 4 cm long segment of the inferior vena cava was replaced by one of the following grafts: cryopreserved aortic homograft (group I, n =5); fresh aortic homograft (group II, n =5); Dacron synthetic vascular prosthesis (group III, n =5).The cryopreserved aortic homografts were stored at -196 ℃ for more than two weeks before grafting. All grafts were retrieved at 8 weeks and assessed by patency rate and histologic observation. Results The patency rates at 8 weeks were 80% in group I,60% in group II and 0 in group III.The cryopreserved specimens before implantation retained morphologic integrity except slight loss of the endothelium. The histological changes seen in the cryopreserved homografts were similar to those of the fresh homografts, including the partial loss of endothelium, shrinkage of smooth muscle in the medium, infiltration of the inflammatory cells in the adventitia. Conclusion Cryopreservation process does not markedly reduce the antigenicityof aortic homograft. The well preserved viability and endothelium of the cryopreserved aortic homograft make it possible to be one of ideal venous grafts.
