摘　　要：目的观察血小板平均体积(MPV)在慢性阻塞性肺疾病(COPD)及COPD合并肺动脉高压(PH)患者中的临床应用价值。方法回顾性分析75例COPD稳定期患者、37例吸烟史匹配的健康体检者,通过心脏扇扫测量三尖瓣返流峰值流速估测肺动脉收缩压(PASP),将其分为3组,即:A组,COPD非PH组;B组,COPD合并PH组;C组,健康对照组。对比各组间MPV均值的差异,并统计MPV与心脏彩超各项指标的相关性。结果相对于C组,MPV在A组和B组升高(P<0.05),在B组升高更加明显(P<0.05)。MPV与左心室射血分数(EF)呈负相关,与右肺动脉直径正相关。结论 MPV在稳定期COPD合并肺动脉高压时升高明显,可作为监测COPD稳定期肺动脉压力及心肺功能受损的潜在指标。Objective To observe the clinical value of mean platelet volume( MPV) in Chronic Obstructive Pulmonary Disease( COPD) and pulmonary hypertension( PH) secondary to COPD. Methods Seventy five patients with COPD and thirty seven controls matched for the age and cigarette / smoke exposure dose were recruited in this study. According to the echocardiography examination,they were divided into three groups( A: COPD without PH,B: COPD associated with PH,C: Controls). The differences between these three groups were compared,and the correlations between MPV values and the indexes of echocardiography were searched. Results Comparing with group C,the value of MPV was significantly higher in group A and group B( P < 0. 05),and group B have a higher level of MPV than group A( P < 0. 05). MPV was negatively correlated with left ventricular ejection fraction( EF),and positively correlated with right pulmonary artery diameter. Conclusion MPV rises during the stable period of COPD and COPD associated with PH,and MPV is correlated with cardiopulmonary function. So MPV may be a good index for the elevation of pulmonary hypertension and an evaluation indicator of cardiopulmonary function.
